The Champions League and Premier League winners have had an offer accepted believed to be in the region of £5m to £6m.
Cardiff also had offers from Premier League rivals Arsenal and Liverpool for the Wales Under-21 star, BBC Sport understands.
Ramsey, 17, has played just 22 games for Championship side Cardiff.
